integrated model of care looks for ways to improve efficiency and
effectiveness: (1) Simulating Kaiser's Advanced Access to
guarantee appointments within three days, (2) Unique interdisciplinary
Plan/Care model – patients coming for episodic reasons treated for all
problems by a pre-assembled team of doctors. |
Focus on use of technology: (1) E-Consults – specialists involved in patient care before scheduling visits, asking health center to administer necessary tests thus saving unnecessary appointments and expediting start of treatment, (2) Project-Eco – educating Community Health Center’s practitioners using weekly video conferences with specialists, (3) Immediate digital access to lab results |
